The coroner Katy Ainge issued a PFD report to the health and social care department on Monday, 20 February.

Katie Wilkins *****: Died at Alder Hey Children’s Hospital- Case Update
The ***** of Katie Wilkins has been partially blamed on a shortage of hematology specialists in the country. On 31 July 2020, the Cheshire girl passed away at Alder Hey Children’s Hospital.
Before she passed out at home on 26 October 2020, Katie made multiple visits to Warrington Hospital during October. She was taken back to the hospital for additional blood work.
The teenage girl was transferred to Alder Hey hospital, where she was diagnosed with coagulopathy, a bleeding disease, and acute promyelocytic leukemia (APML). Katie’s condition worsened, and on 29 July, she had a severe intracerebral hemorrhage.
Due to this, the Cheshire girl underwent emergency surgery but never recovered. Two days later, on 31 July, medical professionals pronounced her dead.
A Government Minister Issued Prevention Of Future Deaths Report
Concerns regarding Katie’s care at Warrington Hospital and Alder Hey hospital were voiced during an inquest held in May last year.
Kate Ainge, an assistant coroner for Liverpool and the Wirral, concluded that this “neglect” was a factor in her *****. Due to that, Ms. Ainge issued a PFD (Prevention of Future Deaths) report to the department of Health and Social Care.
According to a report released on Monday, 20 February 2023, Katy Ainge said that “the inquest brought light to persistent concern that Alder Hey Trust’s primary consultants for the care of APML patients will continue to be Cancer specialists.”
The report continued, “the risk of significant bleeding brought on by the severe and related coagulopathy poses the most significant fatality risk in such patients.
A consultant hematologist who spoke at the inquest as an expert witness and was backed by the trust’s top hematologist agreed that a hematologist should oversee coagulopathy management to prevent more deaths caused by this problem.
Moreover, the report also blamed the incident on the lack of hematologists in the nation, which left Alder Hey trust without resources to address the matter of concern.

Katie Wilkins’ mother, Jeanette Whitfield, and Father, Jonathan Wilkins, spoke with the ECHO shortly after the inquest’s conclusion last May.
The heartbroken mother said her daughter was a fun and sassy girl who loved makeup. “she was bright, caring, and cheerful. She had a big heart and was a tremendous light. Her laugh was so recognizable that I could hear her from a mile away,” Mrs. Wilkins added.
The family said the inquest was very difficult to hear. But they were grateful to the coroner and witnesses for helping them to discover the truth about what happened to their precious daughter.
